Dublin Resource Center Offers Information, Support

On a typical day, Leilani Stone, R.N., nurse health educator at the Dublin Center’s Community Health Resource Center (CHRC), may be helping a patient select a book about managing his or her diabetes, or providing a woman with private consultation on how to perform a breast self examination.
These services are among the many provided by the CHRC to help patients and members of the community lead healthy lives and better understand their medical conditions and diagnoses.
"All of our materials and bookmarked Web sites have been reviewed by PAMF physicians and other health care providers and are from reputable sites, such as the Mayo Clinic and the National Institutes of Health," said Stone. "We offer educational materials and support, and can direct people to resources available at the Dublin Center, as well as local resources in Alameda and Contra Costa counties."
Stone provides both individual and group education, including guidance on how to complete an advance health care directive, and manages the CHRC’s collection of materials and multimedia content, including health-related videos that patrons may borrow. Modeled after the successful resource centers at PAMF’s Fremont and Palo Alto locations, the Dublin Center’s CHRC also offers a free monthly lecture series on a variety of health topics by physicians and other health care providers.
The Dublin Center’s CHRC is open Monday through Friday from 9 a.m. to 3 p.m., and guidance is provided at no cost on a drop-in basis or by appointment. For more information about the CHRC, visit www.pamf.org/health/chrc, or call (925) 875-6150 to schedule an appointment.
