PAMFOnline recently implemented an online process for parents requesting access to their children’s online health record. To request access online, the following criteria must be met:

• The parent is enrolled in PAMFOnline
  
• The parent is making the request
  
• The child is less than 12 years of age
  
• The parent has the legal authority to access information and make medical decisions on behalf of the child
  
• The parent and child live at the same address

If all of the above are met, the parent can make the request online from his or her own PAMFOnline account. It takes just a few minutes. View a step-by-step demonstration online.

If conditions are not met, the parent must fill out the paper request form available online.

To meet the needs of parents with special needs children, exceptions to the guideline that children must be under 12 years of age will be made on a case-by-case basis.
